Once the decision is made to work together, please:
I will email you a link to the Intake Forms. Please review the documents below (you will be asked to agree to the information below, in the intake forms).
The Intake Form needs to be completed at least 1 full business day prior to your appointment, to give me time to review it. If your appointment is set on a Monday, please have forms completed on the previous Friday. Your appointment may be cancelled if I have not received your form at least 1 full business day before your appointment.
