Once the decision is made to work together, please:

•  I will email you a link to the Intake Forms. Please review the documents below (you will be asked to agree to the information below, in the intake forms).

• The Intake Form needs to be completed at least 1 full business day prior to your appointment, to give me time to review it.  If your appointment is set on a Monday, please have forms completed on the previous Friday. Your appointment may be cancelled if I have not received your form at least 1 full business day before your appointment.
