*How are sliding scale rates determined? Sliding scale rates are determined through a conversation. I do not base sliding scale on income, as I understand many people have financial responsibilities not accurately reflected by their income. I also believe you are the expert on what amount you are willing to sacrifice for therapy vs. not attending therapy because it's a financial hardship. If you, like me, see therapy as an investment in yourself, only you can determine how much you can afford to invest, and how highly you value my services (built on years of education and training).

Right to a Good Faith Estimate
You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost. Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.
You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.
Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.
If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.

For the 4 insurance companies above, make online appointments through Headway. Headway does admin and billing for me for these companies, and have different fees than my private practice fees listed at the top of this page.
Payment is due on the day of appointment. Depending on what you have for insurance, or if you are private pay, you may need to agree to place a credit card on file with IvyPay (I will need your mobile phone number) to ensure payment of services. Your credit card information in IvyPay is kept in a secure location, and will be billed on the date of your appointment.
There is a $35 fee for appointments that you do not show up for, or are cancelled with less than 24 hour notice. There are exceptions for illness or emergencies. Please reschedule, and rest, if you are sick.
Miscellaneous Fees
Copies of records: $0.10 per page
Legal Proceedings: $1,500.00 for one-half (1/2) day, to be paid 5 days in advance for court appearance or deposition, and $350.00 per hour for travel, correspondence, and prep time. Clients are responsible for payment, as insurance does not cover these fees. Please see informed consent, which outlines the possibility for termination of therapy, if clients choose to include me in legal proceedings, since this would be considered a dual relationship.
